The game you wanna extract must have a track03.iso and a track05.iso. So, when you use "extract.exe" you have to type:
"extract track03.iso track05.iso (LBA number of the track05.iso)".
Your problem is that you must not have type the LBA number of the track05.iso or a wrong one.
To know this number, you could read it when you use "Tera Term Pro", just before ripping the "track05.iso".
Understand?

It is impossible!
If you have a track03. iso AND a track04.raw there must have a track05.iso, or a lot of track??.raw and a last track which is an ".ISO".
So, look for other tracks; one or severals are missing!

Try to use the "Dcrip.ttl enhanced by Psycomantis" with Tera Term Pro. So, you will be able to choose which track to rip and that way you won't have to rip all tracks.
I am sure tracks are missing because if you have ripped Quake 3 (PAL) there are 17 tracks. And if you have ripped Quake 3 (USA) there are 17 tracks too! (I have just looked... i have the 2 versions of this game).
So, retry to rip this game using the "Dcrip.ttl" i told you and normaly you will see that there are more than 2 tracks.
