Not exactly sure what you are trying to do so I hope this helps.

1. if it is a PC game then I assume you mean that the patch you have is a fixed .exe of some sort.
if this is the case then all you need to do copy the FILES from the CD to a newly created directory of your choice. Then simply search through the "TEMP" folder you created until you find the .exe you have to replace. copy, paste and replace, that's all.
Remember to check the volume name of the CD using windows explorer etc, and if you want to be very good, check it in DOS as well and keep a note.
Place your original in the writer and in nero goto CD-recorder and click on CD info. look at the mode of the first track and make a note of that too.

Then put in a blank and choose a new iso cd compilation. Drag and drop the CONTENTS of the temp folder, not the temp folder itself, across to the compilation. best way is to open the temp folder in the explorer on the right and then right-click and select all, then drag and drop.

open the write CD dialogue box and click through the tabs and change the mode to that of the original and change the volume names, ISO and joliet to that of the original as well. un-tick the box that says about keeping joliet and ISO names the same. Delete the NERO_BURNING_ROM bit.

Goto the burn tab and set the speed and burn.

if you found that the CD has audio tracks after the data track then you need to save the tracks to another temp folder, call it "audio". you can do this by going to cd recorder and save track in NERO. this is easy enough to. if in doubt, consult the help files.

Instead of creating an ISO cd, create a mixed mode CD. again drag your files from the temp folder into the DATA window, and then drag your audio tracks from the audio folder into the AUDIO window of the compilation.

then make sure the volume names etc, are the same and burn.

I think thats it.

A bit of overkill I know but it depends on how good the patched .exe is.

2. if you are doing PSX games, download the ppf patch system from the GCW site and read the help files.

if this is confusing I am sorry.

mail me direct for help.
