It depends what we consider realistic. Can characters from the Sci-Fi world be realistic? When it comes to MMO games in which I play, Warframe has the most refined models (no wonder, you buy Warframes for platinum, and platinum you buy for money). But when it comes to human models, it's definitely Max Payne 3.
The Witcher 3 also has brilliant graphics and character animation.

For real chars i think X-Files The Game, cant be more realistic because it was actually a interactive movie game . It was also a good game (at least for me in my childhood).
I think latest Tomb Raiders are pretty good in visuals and characters.

Another one that is very realistic for pc is Quantum Break, i really enjoyed that game.

For now I would say Death Stranding has the most realistic looking character models. They almost look like their reallife counterparts.

The PC game with the most visually realistic characters is likely Red Dead Redemption 2, showcasing exceptional character rendering and attention to detail.
