Not sure what you mean, but I guess it would be whatever you name the actual image when you make it... lol
 Example: you_file_name_here.mds Note: Remember to load the mds as opposed to the mdf file, or you may have issues

Not for Safedisc. You can load both .MDF and .MDS for safedisc and not have problems because you do not need the DPM, which is stored in the .MDS. For all other protections, you can only mount the .MDS.
