Do you have a non-selfboot game (with an ip.bin in the root directory) and with only 1 file containing "CD001" ?
If so, put all files in a directory, make a 9660 image using CDRWin et Bin2Boot it. Since the image you Bin2Boot-ed contained no ip.bin, it was allready selfboot, so you might have missed some bin files that needed to be hacked.
