Conversion problems

I'm new to this, but I've been following Sgt. Dan's guide. I cannot get AddzerO to work, an error message pops up. In WinXP, it says 'File error 5 - Operation unsuccessful' and in Win98SE I get the message 'GS2patch registry entry for XXXX.ocx file not installed properly.' I'm using AddzerO Forward, with the GUI, but it's a no-go. Sgt. Dan's website isn't up, so I can't ask.
Also, do I need a GameShark to play back-ups, or a mod-chip? Can I play patched back-ups without the mod or GameShark?

you need a modchip, and depending on the chip, an AR2/GS2 or region X too.
