What we do for CD-2-DVD conversions is copy the whole disc to a Temp folder on the hardrive, I usually name my Temp folder the same as the label of the original DVD. This is mainly just for ease so I can copy and paste the name of the folder to the burning program as the label.
Add folders for your Trainer, Saved Games etc.
Also inlcude a Crack folder and copy the correct NoCD crack to the Crack folder.
And yes you can just skip the 00000001.tmp and 00000002.tmp files, they are part of the protection and since you wont be making a 1:1 copy, and will HAVE to use a NoCD crack to play, you wont need those 2 files as part of your DVD.
Burn the contents of the Temp folder to a DVD, naming your DVD the same as the original DVD.
Just use Nero to burn, burn the disc as a Data disc.

The DVD will work to install the game only, then use a NoCD crack to play.
