I know this is an old topic, but in case anyone is still wondering about the question... Gameshark checks the version number in the IP.BIN, so if you have a rip from someone who uses a general IP.BIN for their games, (such as using a Crazy Taxi IP.BIN for all their rips), the version # may be 1.000 for example, but if the original GDROM is 1.001 or something different, it won't work. Sometimes there were even multiple version #'s released by the developer, such as Sonic Adventure.

Another reason could be that a lot of the pirated releases were of EU/PAL versions of the games, but you may not realize it. Red Dog (from echelon), for example, was an EU version release, but looks just like the US release.
Codebreaker also checks the game name and version number in IP.BIN. Because inputting codes on it is such a pain, I actually wrote a problem which takes game codes as a script and spits out a DCI that you can upload to a VMU already full of your codes. Though it needs some slight editing first, so it's not really any good as a general public tool yet.

Still can't find that bootloader thing anywhere.
