1. Power on the xbox with the eject button. Don't close the tray.
2. When the dashboard loads up, put the game in the tray and close it. 3. Let the xbox read the game, then ftp. When you ftp, the game should be shown in the d drive. Auto-launch has to be set to off, which you already said it was, so I wouldn't worry about that.
