Do a thorough search of your disc, look for little scratches on the label side of your disc. If you have a scratch on that side your disc is ruined.
I have come across this once. It was so small I could barely see it. I had to test it by touching it my my felt tip marker and when I saw that it blead through I knew there was no fixing it. Tyler!
