How to create multi session cd's with one track bootable? (see SHENMUE 2)

Hi all.
I am trying it cause my Shenmue's second cd is corrupted...
So I have just CRC'ed data files but I can't make a multisession cd that contains in one of the data tracks the bootsector(IP.bin)
Example stuff:

1st session:
track01 (mode 2-XA) LBA 0
2nd session
track02 (mode2-XA) LBA 45000 (this track must contains
the bootsector and have
indexes that refers to the 1st
track files)

The issue is that i can make multisession cd with the second track that contains the boot sector but i can t make its indexes referring to files in the first track.
So how it is possible to reproduce a cd like the shenmue2 one is made?
