There are patches to apply to both disc's. It sounds to me like you only applied the patch to the first disc, or applied the same patch to both discs, when in fact there are seperate patches for each disc.
Check out the pdx patch (pdx-d2, it's 900kb) on psxcopyworld, you'll find there's two patches in them One to be applied to each disc.
