I found that whevever I changed any of games settings I had the same problem you seem to be experiencing. The game for whatever reason failed to load. I had to completely uninstall the game and start over again from scratch. I recall this game takes foreeevvveeerrr to be installed. If you apply your patches and under no circumstances change any of the game settings, either visual details or sound details you should be okay. I don't know about the differences between international or domestic patches.
