Modchip, Help!

I have been given a Playstation by a friend, it had a modchip fitted, but unfortunately the chip has come away from the board due to poor soldering. What I need to know is what kind of modchip it is, so that I can search for a diagram to re-solder.
The modchip in question has 4 prongs either side and has this info printed on the front, 12C508A

it also has 4 wires which are no longer connected to the chip.
The playstation is model 900x.
Any help would be much appreciated,
