How do I use ISOBuster ???

I want to edit my CloneCD image file........ I want to insert a file into it. So waht do I do now ?? I open ISOBuster, then I open my image file (operation.img). But I don't know what I have to do now. Do I have to uncompress it again ?? Could anybody help me ?? Thanx !
