OK, so I'm a bit dense, MCM2 assistance please?

I downloaded and unzipped the patch with the executable that I am supposed to use to replace the one in the image file with. However, I cannot see the MCM2.exe in the image file. How do I do this replacement. I'm sorry if this is ultra simple for some, but it is just not clicking for some reason. I'm new to CD copying though if that helps excuse my dense gray matter.
