Sorry I havn't got anything more substantial, but here's the recommended procedure for conneting cd-roms and hard drives:
IDE PORTS:
Primary: master = hard-drive
slave = not connected or usually your older, slower hard-drive
Secondary: master = cd-rom
slave = writer
So you'll be using two IDE cables - one for your hard drives and one for your cd-rom/writer.
I don't know wether using DAM-66/100 cables and IDE ports would affect the performance of your machine, but it's just a thought.
I guess yo probably know these settings, but at least we can rule this out from now on.
How about trying the drive in a friend's pc (ideally one with a writer)? If the drive fails again then could you take it back to the shop?
Shame this drive is playing you up, as you otherwise seem to have a pretty decent pc setup.
All the best.
