Dunno what AOpen is but it doesn't seem a good brand to me; use Pioneer or LG instead. If it doesn't read your original discs, contact the publisher - although, I'm sure they will tell you exactly what I wrote: buy a new DVD-ROM drive.
As for creating an ISO (from your original disc, as the poster you've quoted wrote), read the forum FAQ for backup guides. "[MULTI]" means that the game can be played in multiple languages; "[MULTI4]" means 4 languages (my guess: English, German, French and Spanish). Yes, an MDS file is that small as it contains only structural information from the original disc. Create the accompanying MDF file yourself, with Alcohol 120%, from your original disc and put the two files together; NOW you have a working image that you can burn or mount. Nope, Nero is not good for any step of backing up copy protected stuff.
