No with the CC1.0 you do not use the TO point. It appears in one of my V14 installs as it was mistakenly added. That picture was taken before I had tested it, notice the wax paper. This is a CC1.0 in a V14

No you do not need the CC1.0 preflashed with R18 but if you have it flashed with anything other then a bios compatible with V14's this might be your problem. The factory flash is blank and boots the chip from recovery mode. Have you tried booting the chip in recovery mode and then running the upgrade disc? To do so tap reset 3 times in a row. Read here
