Could be anything. Some examples of common causes: Overheated machine, Scratched/smudged/cracked disc, dirty laser lens, scratched laser lens, damaged hardware (any part of the disc drive or its linkage to the processor), etc. etc. etc.

1. Make sure your disc is clean and in good condition (no scratches, etc.)

The first thing to try after that would be to take a cotton swab and just dampen it a LITTLE and then wipe off the laser lens gently, no need to press and scrub. Dry it with the other end and try the machine. do this VERY carefully as the lasers in PS2's are extremely delicate and prone to damage. (Caveat Emptor: It's not my fault if you damage your machine.)
