Thanks for the reply. Yes, I put the contents of CD4 into the same temp folder and named all volumes the same as per the intructions in this post. Also the image name inside Ultra ISO was the same as in the MSI installer.

Strange thing is that I had no problems with Doom3 and the expansion. I used the guide from this site... weird.
