1st check for scratches on the media, i dont mean general use scratches, but big ass deep ones.
2nd if none, copy the contents of the disc to a computer, if the files copy over a good chance the disc is fine.
3rd try asking people when it happens, and try it yourself. As some home players etc are ficklesome with types of discs, even silver pressed ones.
BTW have a check-in checkup service, when they return check the media for any obvious tampering (also before giving out) this way you can easily check who has tampered with it.
As most kids, will get a game copy it, ruin it, so no-one else can use it.
