It might seem obvious, but some people don't realize that you need to make sure the internet connection is off! It won't go to off-line mode as long as the connection is on...

On my end, I have to make sure that it's off when booting up my PC. Otherwise it seems to know that I have a connection, whether it's on or off at the time I start the game doesn't matter. Basically, when loading the game if it detects the connection, it either just hangs if it's off, or it connects to Steam if it isn't. That said, If done correctly it will appear that it is connecting, but you will get an error that asks if you want to play off-line or try connecting again. Choose off-line and it should work. Good luck!

BTW, I have an external cable modem. Not sure what someone with an internal modem can do in this regard!

From what some people say about the EULA, I would assume that there is now no need for the CD/DVD patch at all, and it would be safer just not using it. It surprises me there are many still looking for such patches, let alone people using the patch knowing of the update...
