I have actually started using the pen too and find it to work very good. But I did countless vias by just scrapping with a brand new exacto knife before I got the pen and it worked just fine. I also like to use the paste as warlock suggested as there is more control over where it goes.

Puppet you have been very lucky in not needing to remove the lacquer. I have seen vias on V9's that needed very little removal and others that there was no copper showing at all on the via. Every board has its differences even when they are the same version.
