www.foundmy.com is a great place to purchase mod chips from. I and others on this forum have used them with success. Modchips.ca is VERY expensive.
As for what you get with the chip depends on where you buy it from. If you mean like wire and solder and that, it’s not worth paying a high price for this with the chip when you can get it at Radio Shack for way cheaper, and you will more then likely be having to make a trip to radio shack to get some flux (and probably an iron if you have never soldered) any way as I have never heard of a chip seller putting that in the bag with the chip.
