Ask your installer to ensure that there is a 2.5mm or 3mm heatsink pad on the LA chip. It is important to conduct heat away because I have proved to a reasonable degree of certainty in an experiment that DVD-R (and thus DVD+R) of imperfect quality can seriously stress that chip and cause it to blow.
I would love to post the full experiment results here - but it requires a 256K image in support that i cannot upload.
Bottom line is that many v9s come without a heatsink on the LA chip that controls the focus, tracking and sled movement. The LA chip is rated (if it is like the BA chip on earlier models) to dissipate 2.2W at 25°C. Everyone knows that in the parts of the PS2 that the fan doesn't reach, the temperature will rise - probably to about 60°C. In that case the power dissipation threshold is much lower and certainly exceeded if the laser has to deal with poor quality DVD-R media.
Unless a heatsink is fitted like it used to be on the v7s and earlier.
So that's my advice if you want to avoid potential problems downstream.
