Got an email from Playstation Underground saying if i fill out this online survey they would send a free PS CD case.

You cant enter the same address twice so you need to change the last 3 digits(750) to a random number. to be able to apply for another free case. If yo ucahnge the last 3 numbers and dont get a valid page then enter some new numbers. I hope this allowed, if not please delete.
