You need to make sure you're not copying disc-to-disc as this is not recommended.

You need to build up an image of the disc you're copying first, on your hard drive; once the image is created, your computer can supply the writer with a continous stream of data and hence avoid the buffer underrun problem.

Also note: the ability to make 100% backups depends on what hardware you have (cd-rom/writer) and what copying software you've got installed. The help file in CloneCD has a big table of compatible drives, and this program is very popular amongst the backing-up community, for all types of stuff!
