You can use gimp or photoshop.

1 block from the top is for Normal when not Checked;
2 block from the top is for selected when not Checked;
3 block from the top is for Clicked when not Checked;
4 block from the top is for Disabled when not Checked;

5 block from the top is for Normal when Checked;
6 block from the top is for selected when Checked;
7 block from the top is for Clicked when Checked;
8 block from the top is for Disabled when Checked;

Don't worry, you have this worries in EZ Installer on how to create that, I'll make a plan. :D
