Requesting New DVD from Activision for CoD:WaW
 

Yes, I realize the game came out less than a month ago but I have already lost my DVD. I saw in the rules there was a way to ask Activision for a new copy of my DVD for a small fee, I was just wondering how I might go about requesting one. Thanks.
