Would it be possible to make your own boot/swap disc?

NOPE......

The information that the PS2 uses to determine if its a Original or a Copy is in the pre-groove area of the disc....

There is not a burner available that can replicate this information.. Because that info is not "written" to the disc, it is "pressed" on the disc when manufactured.....

This is the reason a mod chip is needed.... It "fools" the PS2 into thinking that you have inserted a Original instead of a Copy..

Then what do those boot discs do? Isn't there some modchips that use something like that? :o

I guess the only boot disc you could make would require the original press copy of the game, at which point you'd just make the game itself.

That will give you a idea of how they work...
NOTE: Even with the boot discs, you still have to have a mod chip..
