How do i apply a patch to an iso??

When backing up games such as Cart Fury (which is a dvd) how do you apply the patch once you have made your iso file. I noticed that the Patch programs look for .bin files to patch not .iso files. I know how to create a bin file but because this particular game is a dvd, i cannot just burn the patched bin to a cd. It needs to be edited first. How do i patch a game of this type?
